PC-PNDT Form F software: prepared while you report, not reconstructed at month-end
By Dr. Shashikant Morale, radiologist · Updated August 2026
Every sonologist in India knows the Form F evening: the month's registers open on the desk, the portal open in a browser, and hours of re-typing information that was already written down once — under the pressure that an omission in a statutory record is not a small mistake. MEERAVIO's position is simple: Form F should be a by-product of reporting, not a second job.
What Form F is (in one paragraph)
Under the PC-PNDT Act and its Rules, every registered ultrasound/genetic clinic must maintain a Form F record for each pregnant woman undergoing an ultrasound examination — her details, the referring doctor, the indication for the scan, and the declaration that sex determination was neither sought nor disclosed — and submit these records to the Appropriate Authority, typically monthly. Incomplete or missing forms are treated seriously, which is why they deserve better than end-of-month reconstruction from memory and registers.
How MEERAVIO handles Form F
- Filled from data you already entered. Registration and the report already contain what Form F asks — identity, referral, indication, LMP. MEERAVIO assembles the form from that data the moment the report is done.
- Required fields checked immediately — while the patient is still at the clinic, not weeks later when the portal rejects the entry.
- Portal auto-fill, doctor-controlled. MEERAVIO can fill the government portal's fields for you. It never logs in by itself, never touches the CAPTCHA, never presses Submit, and never overwrites anything you typed. You review, you submit.
- Machine-wise records, automatically. Each scan is recorded against the specific ultrasound machine that performed it (from the machine's DICOM identity) — the shape in which inspections actually ask for data.
- An attestation gate before anything leaves. The same safety gate that stands before report export stands before Form F: the physician confirms the content, and that confirmation is logged.
Why the "doctor submits" design is deliberate
Form F is a statutory declaration carrying the doctor's responsibility. Software that silently files it would be doing something no practitioner should delegate — and would break the moment a portal changes. MEERAVIO's division of labour is fixed: the software eliminates re-typing; the doctor exercises judgement and files. That is also why AI-drafted wording anywhere in MEERAVIO stays visibly marked until the doctor has edited it.
What you need
MEERAVIO runs on a Windows 10/11 PC on the same network as your ultrasound machine. Form F preparation works with the data you capture in normal reporting — there is no separate data entry. Try everything free for 14 days.
